Preop: 300-600 mg PO 1-2 hr before surgery. Multimodal analgesia. Reduces opioid consumption. Causes sedation and dizziness. Caution in elderly/OSA.

- Severe (eGFR <30 / dialysis)Renally eliminated — substantial reduction required if CrCl <30; additive sedation/respiratory depression with opioids.
