Renal & hepatic adjustment
- Severe (eGFR <30 / dialysis)Active metabolite (3-desacetylvecuronium) accumulates in renal failure → markedly prolonged block. Avoid repeat dosing; cisatracurium is preferred.
- Severe (Child-Pugh C)Hepatic and renal elimination — prolonged block in hepatic dysfunction; prefer cisatracurium.
