Renal & hepatic adjustment
- Moderate (eGFR 30–59)Active metabolite M6G begins to accumulate — reduce dose / extend interval and monitor sedation.
- Severe (eGFR <30 / dialysis)M6G accumulates substantially in severe renal impairment → prolonged sedation and respiratory depression. Reduce dose, extend interval, or use fentanyl/hydromorphone with caution.
- Severe (Child-Pugh C)Reduced hepatic clearance — reduce dose and extend the dosing interval.
